By means of this form, I exercise my right to deletion, in accordance with the provisions of Article 17 of the Regulation (EU) 2016/679 of the European Parliament and Council of April 27, 2016, regarding the protection of natural persons with regard to the processing of personal data and on the free movement of such data (RGPD), and request that the Company proceed with the deletion of all personal data of the affected person (or of the person represented) for the following reasons:
